·         CDM offers employees the choice of coverage benefit plans designed to meet ACA requirements.

HMO & Added Choice

·         Coverage includes employee, spouse, and any dependents up to age 26.

Assurant

·         If dental insurance is elected, employees are responsible for a minimal premium contribution.

Flexible Spending Account(FSA)

·          Allows employees to set aside a specific pre-tax dollar amount for unreimbursed medical, dental or dependent care expenses.

Life Insurance: SunLife

·         Employees are eligible for a flat $15,000 basic life and AD&D benefit

·         In addition, employees have the option to purchase additional life insurance on themselves (up to 150,000), spouse and/or children.

 Voluntary Short Term/ Long Term Disability

·       Insurance premiums are based upon an employee’s age and base salary amount
